Set up the driver booking form

The booking form asks drivers your own questions when they book: the carrier, the truck's plate, the PO or bill of lading, the load. It's designed in Studio, like BookIt ONE's. See Design the booking form.
Prerequisites
- The Admin or Designer role.
- The Professional edition, for Studio.
Show the carrier and plate at the gate
The gate, the dock board and self check-in show the carrier and plate from two questions of the form:

A field's key follows its label until the form is first published, and is fixed after that. Add these questions labeled Carrier and Plate, publish the form, and then rename them if you like, such as "Truck license plate": the keys stay. If the keys are different, the gate shows No plate recorded.
Tip
Add a rule to the form to require a PO or bill of lading for full truckloads, or to send hazardous loads for approval with Needs approval.
